Miranda is the daughter of the Australian actor, Barry Otto. She originally intended to go into medicine, but decided to move into acting instead. After graduating from the NIDA Theatrical School in 1990 she made her film debut in Emma's War (1986), gaining recognition in The Girl Who Came Late (1991) and and Gillian Anderson's Last Days of Chez Nous. She has been in many critically acclaimed but little-seen films.

After a decade of powerful roles in Australian films she finally got Hollywood's attention with small roles in films such as The Thin Red Line (1998) and as the mysterious neighbour Robert Zemeckis' What Lies Beneath (2000).

After Cameron Diaz decided to star in Charlie Kaufman's Being John Malkovich, Otto lost the role. However Kaufman later wrote Human Nature, in which she played the role of Gabrielle, much to her delight. Her role as an affectionate French lab assistant showed her comic skills were as good as her dramatic skills, and it wasn't long before the film offers were rolling in.

She was cast the role of Eowyn in the Lord of the Rings trilogy, playing Tolkien's only really detailed female character in the books. She spent six-months on set and had to learn sword fighting and horseback riding for the role.

She next appeared in two largely unseen films, Hynotic and Julie Walking Home (2002), despite her moving performances. In 2003 she teamed up again with Rhys Ifans to play a comic role in Danny Deckchair to great success and also starred in "The Three Legged Fox", directed by Sandro Dionisio, as well as "Doctor Sleep," a thriller that also stars ER's Goran Visnjic. In 2004 she played Her latest role in the Flight of the Phoenix about the story of the survivors of a plane that crashed in the Mongolian  desert, and In My Father's Den.  2005 saw her playing a role in War of the Worlds (2005) which looks set to be another Hollywood blockbuster!

She has gained Australian Film Institute award nominations for her work in "In The Winter Dark," "The Well," "Daydream Believer," and "The Last Days of Chez Nous." She also earned an Australian Film Critics Circle Award nomination for her performance in "Last Days of Chez Nous," as well as for "Love Serenade," which won the Camera d'Or at the Cannes Film Festival.

She met her husband, Peter O’Brien in 2002 while performing A Doll's House on stage in Australia and they were married in 2003.

Gave birth to her first child, a girl, named Darcey in Australia. Her husband, who was unable to attend the birth, watched via web cam from America.
